nca: fix impl issues in ncaInitializeContextCommon()
Other changes include: * cnmt, nca: handle NcmContentId elements in a more uniform way. * gamecard: add some comments to the GameCardFlags enum. * nxdt_log: make sure log messages are horizontally aligned.
